N. Blaine Cook, DDS, MSD
Dr. Cook received a DDS degree from the University of Pacific School of Dentistry in 1981 and practiced general dentistry in Idaho Falls, Idaho for 5 years. In 1986 he entered the U.S. Navy Dental Corps. While in the Navy, he received postdoctoral training in Operative and Preventive Dentistry at the Indiana University School of Dentistry and was awarded a Master of Science in Dentistry (MSD) degree in 1997. He was certified by the American Board of Operative Dentistry in 2000. His final naval tour of duty was at the Naval Postgraduate Dental School at Bethesda, Maryland where he was the chair of Operative Dentistry and the Specialty Leader for Operative Dentistry to the Navy Surgeon General. He retired from the Navy in 2006 after 20 years of service and joined the dental faculty at the University of Nebraska Medical Center College of Dentistry where he taught for 3 years. Dr. Cook joined the Indiana University School of Dentistry faculty in 2009 to become the director of the Graduate Operative Dentistry residency program. He is an associate editor for the Operative Dentistry journal, and an active member of the American Board of Operative Dentistry, the Academy of Operative Dentistry, the American Dental Association, the American Dental Education Association, the Indiana Dental Association, and the Indianapolis District Dental Society.
